Match Green and White Form document routes
Use actual official documents to identify the relevant route before selecting its guide and forms.

Identify the documents first
Green Form status documents, White Form application records and secondary-market purchase certificates serve different purposes. List their names, applicable scheme and dates and confirm unresolved route questions with the relevant office.
Use the matching HOS/GSH Green Form guide or HOS White Form/WSM guide. Closed-year material can support an existing application without establishing that new applications are open.
Separate route and task
Check the actual document wording.
| Item | Record | Follow-up |
|---|---|---|
| Scheme | Name and year | Choose the matching guide |
| Status document | Name and scope | Confirm with the issuer |
| Form | Green or White Form version | Match the application |
| Next step | Notice and handling office | Keep follow-up records |
Example: a document names a specific scheme
A status document naming one scheme should not be applied automatically to another transaction merely because both use a Green Form label.
Keep the route consistent
- Document names and dates are clear
- Guide years match
- The authority resolves status questions
